About the role:
This Nurse Practitioner role will be providing the full scope of NP primary care services to clients accessing services. Working as part of the Indigenous interprofessional primary health care team, the Nurse Practitioner will provide trauma-informed, culturally safe primary health care to full scope with a strong emphasis on health promotion and disease prevention for Indigenous patients. The Nurse Practitioner will assess, diagnose, treat and monitor a wide range of health problems while engaging patients as full partners in their care plan.
Qualifications:
- Member in good standing and without restrictions, College of Nurses of Ontario (CNO) as a Registered Nurse in Extended Class [RN(EC)] required
- Member of the Registered Nurses Association of Ontario and/or the Nurse Practitioners Association of Ontario
- Baccalaureate Degree in Nursing from a recognized University and Certificate as a Primary Health Care Nurse Practitioner

Position Overview:
The Nurse Practitioner will independently assess and provide comprehensive care in a virtual clinic environment. You’ll be responsible for diagnosing conditions, prescribing medications, ordering diagnostic tests, and managing treatments, while collaborating with healthcare professionals when necessary. You will be expected to deliver holistic, patient-centered care that includes health promotion, injury and illness treatment, and preventative strategies.
Essential Duties:
- Assessing client presentation, evaluating urgency of care needed, selecting appropriate level of care, facilitating referrals to other providers where appropriate, and determining follow up required
- Independently and collaboratively assesses, diagnoses, and prescribes in accordance with professional standards, legislation, policies, and clinical practice guidelines
- Coordinates treatment plans and follow up based on appropriate assessments, client and family response and outcomes, collaborating with other healthcare professionals if needed
- Communicates findings and diagnosis to client and/or inter-professional team. Discusses prognosis, treatment options and outcomes with client and/or other team members within the circle of care and documents within the client’s permanent health record

Qualifications:
- Graduation from a Nurse Practitioner program

- Preferred 1 year of recent experience as a practicing Nurse Practitioner preferred. Previous clinical experience will be considered
